Who We Are: Guiding Hands Senior Placement is a senior care placement agency operating in Arizona. We help families identify and connect with appropriate assisted living, memory care, and other senior care communities. We are not a HIPAA-covered entity; however, we voluntarily adhere to strong privacy standards to protect all information you share with us.
Information We Collect: In order to provide effective placement services, we collect personal information including name, date of birth, contact information, health and medical history, activities of daily living (ADL) assessments, cognitive and behavioral information, financial information, and care preferences.
How We Use Your Information: The information you provide is used solely to: (1) identify and recommend appropriate senior care communities or services; (2) facilitate tours, applications, and move-in coordination; (3) communicate with care communities on your behalf with your authorization; and (4) comply with applicable legal requirements.
Information Sharing: We will not share your personal or health information with any third party without your explicit consent, except: (a) with senior care communities directly involved in your placement, as necessary to evaluate fit and availability; (b) as required by law or a valid legal process; or (c) in an emergency situation to protect health or safety.
Data Security: We implement reasonable administrative, technical, and physical safeguards to protect your information from unauthorized access, disclosure, alteration, or destruction. All digital data is transmitted via secure, encrypted connections.
Your Rights: You have the right to: request access to your information; request corrections to inaccurate information; request that we limit how we use or share your information; and withdraw your consent at any time by contacting us in writing.
Retention: We retain your information for as long as necessary to provide our services and comply with applicable legal obligations, typically no more than 7 years following the conclusion of our engagement.
